Pediatric Referral Lists:

ReferralsReferrals

While we provide comprehensive care to our patients, situations arise when seeking outside help from a subspecialist becomes necessary. Many insurers create a "gatekeeper" model requiring your child's caregiver to authorize referrals to specialists. We provide these referrals in consultation with you when medically needed and only after a visit with one of our providers. It is your responsibility to ensure that the specialist you choose to seek care from is covered by your insurance provider. When making an appointment, confirm this information.

Steps to Help Your Referral Process

  1. Check with your insurance company to see who is a participating specialist.
  2. Make your appointment and verify that the specialist is still a participating provider with your insurance.
  3. Some insurance companies require a referral form that must be generated by our practice. Verify this with your insurer.
  4. IF your company requires a special referral form, contact our office with the following information:
    • name of specialist
    • phone number of specialist
    • your appointment time and date
  5. Before being seen you should have received a copy of the referral by mail or if you had requested it to be faxed to the specialist check to see that the specialist has received it. If you have not received it by mail and the specialist doesn’t have it, call our office to make sure the referral has been done. In many cases we cannot backdate the referral.  Please note that any charges due to the lack of a referral will be your responsibility as per the insurance company.
